Common use

Erectile dysfunction is defined as the consistent or recurrent inability to achieve and/or maintain a penile erection sufficient for satisfactory sexual performance. It is used on an as-needed basis to facilitate erections when taken in conjunction with sexual stimulation. It is not an aphrodisiac and does not increase libido. Its use is widespread among men with ED of various etiologies, including vasculogenic, neurogenic, and psychogenic causes, as well as ED associated with common comorbidities like diabetes mellitus, hypertension, and spinal cord injury.
Dosage and direction
- The recommended starting dose for most patients is 50 mg, taken approximately 15-30 minutes prior to anticipated sexual activity.
- Based on efficacy and tolerability, the dose may be increased to a maximum recommended dose of 100 mg or decreased to 25 mg.
- The maximum recommended dosing frequency is once per day.
- Administration Instructions: Place the tablet under the tongue and allow it to dissolve completely. Do not chew, crush, or swallow the tablet whole. Absorption occurs through the oral mucosa. Do not eat or drink anything for at least 5 minutes after the tablet has dissolved to ensure optimal absorption.
- The medication’s effectiveness is dependent on sexual stimulation.
Precautions
- Cardiovascular Status: Patients should undergo a thorough medical history and physical examination to diagnose ED, determine potential underlying causes, and assess cardiovascular health before initiating any treatment for ED. Sexual activity carries a potential cardiac risk; therefore, physicians should consider the cardiovascular status of their patients prior to recommending any treatment for ED.
- Priapism: Prolonged erections lasting more than 4 hours and priapism (a painful erection lasting more than 6 hours) have been reported infrequently. If an erection persists for longer than 4 hours, the patient should seek immediate medical attention, as priapism, if not treated promptly, can result in permanent damage to the erectile tissue and irreversible impotence.
- Effects on the Eye: Non-arteritic anterior ischemic optic neuropathy (NAION), a cause of decreased vision or permanent vision loss, has been reported rarely post-marketing in connection with PDE5 inhibitor use. It is not possible to determine if these events are related to the drug or other factors. Patients who have experienced NAION in one eye are at an increased risk of NAION in the other eye.
- Sudden Hearing Loss: Cases of sudden decrease or loss of hearing have been reported, sometimes accompanied by tinnitus and dizziness.
- Alcohol: Excessive consumption of alcohol (e.g., 5 units or more) in combination with sildenafil can increase the risk of orthostatic hypotension, dizziness, and increased heart rate.
- Renal or Hepatic Impairment: Since sildenafil clearance is reduced in patients with severe renal impairment or hepatic cirrhosis, a 25 mg dose should be considered for these populations.
Contraindications
- Concomitant administration of any form of organic nitrates (e.g., nitroglycerin, isosorbide mononitrate or dinitrate) or nitric oxide donors (e.g., amyl nitrite “poppers”) in any form, either regularly or intermittently. This combination can cause a severe, life-threatening drop in blood pressure.
- Hypersensitivity to sildenafil or any of the excipients listed in the formulation.
- Concomitant use with guanylate cyclase stimulators, such as riociguat. PDE5 inhibitors may potentiate the hypotensive effects of riociguat.
- Patients for whom sexual activity is inadvisable due to underlying cardiovascular pathology.
Possible side effect
Like all medicines, Viagra Sublingual can cause side effects, although not everybody gets them. Most are mild to moderate and transient.
- Very Common (≥1/10): Headache, flushing.
- Common (≥1/100 to <1/10): Dyspepsia, nasal congestion, dizziness, visual disturbances (e.g., blurred vision, cyanopsia - seeing a blue tinge to objects), nausea.
- Uncommon (≥1/1,000 to <1/100): Hypotension, syncope, tachycardia, palpitations, vomiting, rash, dry mouth, eye pain, photophobia.
- Rare (≥1/10,000 to <1/1,000): Hypersensitivity reactions (including skin rashes), priapism, NAION, sudden hearing loss, hematuria.
- Frequency Not Known: Seizure, transient global amnesia.
Drug interaction
- Nitrates: Absolute Contraindication. Concomitant use can lead to profound hypotension, syncope, myocardial infarction, or stroke.
- Alpha-blockers (e.g., doxazosin, tamsulosin): Concomitant use can potentiate the blood-pressure-lowering effects of both drugs, leading to symptomatic hypotension (e.g., dizziness, lightheadedness, fainting). Dose adjustment and careful monitoring may be required.
- Potent CYP3A4 Inhibitors: Drugs like ketoconazole, itraconazole, ritonavir, and clarithromycin can significantly increase sildenafil plasma concentrations. A maximum dose of 25 mg within a 48-hour period is recommended when co-administered.
- Other Antihypertensives: Additive hypotensive effects may occur when taken with other blood pressure-lowering medications.
- Alcohol: As noted in Precautions, heavy consumption can increase the risk of orthostatic signs and symptoms.
Missed dose
Viagra Sublingual is not intended for chronic daily dosing. It is taken on an as-needed basis. Therefore, the concept of a “missed dose” does not apply. Simply take the next dose when needed, prior to sexual activity, ensuring you adhere to the once-daily maximum dosing frequency.
Overdose
- Symptoms: In line with the drug’s known adverse effects, an overdose could be expected to produce severe manifestations of these effects, including severe hypotension, syncope, prolonged and potentially harmful erection (priapism), vertigo, and palpitations.
- Management: Standard supportive measures should be adopted as required. This includes placing the patient in the supine position with the legs elevated to manage hypotension and ensure a stable airway. Hemodialysis is not expected to accelerate clearance as sildenafil is highly protein-bound and extensively metabolized. Management of priapism is a urological emergency requiring immediate intervention to irrigate the corpora cavernosa.
Storage
- Store below 25°C (77°F) or as directed on the packaging.
- Keep in the original blister pack or container to protect from moisture and light.
- Keep out of the sight and reach of children.
- Do not use after the expiration date printed on the packaging.
