Terramycin (oxytetracycline with polymyxin B) is a time-tested, prescription-only ophthalmic and topical antibiotic ointment. It combines two potent antimicrobial agents to deliver robust, broad-spectrum coverage against a wide array of gram-positive and gram-negative bacteria. Clinically trusted for decades, it is formulated to treat superficial ocular infections and prevent secondary bacterial infections in minor skin abrasions. Its vaseline-based ointment ensures prolonged contact time for sustained therapeutic effect.

Features

  • Contains oxytetracycline hydrochloride (5 mg/g) and polymyxin B sulfate (10,000 units/g)
  • Broad-spectrum antibacterial activity covering common ocular and cutaneous pathogens
  • Ophthalmic and topical dermatological formulation
  • Sterile, preservative-free ointment base for minimal irritation
  • Available in 3.5 g tubes with a precision tip for controlled application

Benefits

  • Effectively treats bacterial conjunctivitis, blepharitis, and other superficial eye infections
  • Prevents infection in minor cuts, burns, and abrasions, promoting faster wound healing
  • Dual antibiotic action reduces the risk of bacterial resistance development
  • Ointment formulation provides longer residence time on ocular or skin surface compared to drops
  • Minimizes risk of secondary infections in compromised epithelial barriers
  • Trusted efficacy with a well-established safety profile in clinical use

Common use

Terramycin ointment is primarily indicated for the treatment of superficial ocular infections caused by susceptible strains of microorganisms, including acute and chronic conjunctivitis, blepharitis, blepharoconjunctivitis, keratitis, and corneal ulcers. It is also used prophylactically to prevent infection following minor corneal abrasions or removal of foreign bodies. Dermatologically, it is applied to minor skin wounds, burns, and sutured areas to prevent bacterial contamination and support clean healing. Veterinarians frequently use Terramycin in similar contexts for ocular and topical infections in animals, though human formulation should only be used under medical direction.

Dosage and direction

For ophthalmic use: Apply a half-inch (approximately 1.25 cm) ribbon of ointment into the affected eye(s) every 3 to 4 hours, or as directed by a physician. Gently pull down the lower eyelid to create a small pocket, apply the ointment, and close the eye for 1–2 minutes. Rotate the tube tip to help spread the medication. Avoid touching the tip to any surface, including the eye, to prevent contamination.

For topical use: Cleanse the affected skin area gently and dry thoroughly. Apply a thin layer of ointment to cover the wound or affected area 2 to 4 times daily. May be covered with a sterile bandage if desired.

Duration of treatment typically ranges from 5 to 7 days, or as advised by a healthcare provider. Even if symptoms improve, complete the full course of therapy to prevent recurrence or resistance.

Precautions

  • For external use only. Do not ingest or apply inside the mouth or nose.
  • Do not use in the eyes if wearing contact lenses; remove lenses before application and wait at least 15 minutes before reinserting.
  • Avoid sharing the medication tube to prevent cross-contamination.
  • If condition worsens or does not improve within 48 hours, discontinue use and consult a physician.
  • Prolonged use may result in overgrowth of non-susceptible organisms, including fungi.
  • Use with caution in patients with a history of allergic reactions to antibiotics or ointment bases.
  • Pregnancy and lactation: Consult a healthcare provider before use; oxytetracycline may affect fetal development or pass into breast milk.

Contraindications

Terramycin is contraindicated in patients with known hypersensitivity to oxytetracycline, polymyxin B, or any component of the formulation. It should not be used in viral or fungal infections of the eye or skin unless superinfected with bacteria susceptible to its components. Avoid use in deep puncture wounds, severe burns, or serious skin conditions without medical supervision. Not recommended for children under 8 years of age unless specifically indicated and supervised by a physician, due to potential effects on developing teeth and bones.

Possible side effect

Common side effects are typically mild and may include temporary blurred vision, mild stinging or burning upon application, eye irritation, or redness. These usually subside as the eye adjusts to the medication. Less frequently, patients may experience itching, swelling, or dermatitis at the application site. Hypersensitivity reactions, though rare, can include rash, urticaria, facial swelling, or difficulty breathing—discontinue use immediately and seek medical attention if these occur. Superinfections with non-susceptible organisms may manifest as persistent or worsening symptoms.

Drug interaction

Oxytetracycline may interact with oral anticoagulants (e.g., warfarin), potentially enhancing their effect and increasing bleeding risk. Concurrent use with other topical products (e.g., corticosteroids, other antibiotics) is not recommended unless directed by a physician. Oral tetracyclines can reduce the efficacy of oral contraceptives; though topical absorption is low, caution is advised. Avoid using Terramycin concomitantly with bacteriostatic antibiotics, as it may antagonize the bactericidal effect of polymyxin B. Inform your doctor of all medications, including over-the-counter products and supplements.

Missed dose

If a dose is missed, apply it as soon as remembered. However, if it is almost time for the next scheduled dose, skip the missed dose and resume the regular dosing schedule. Do not double the dose to catch up. Consistent application is important for efficacy, but occasional missed doses are unlikely to significantly impact treatment outcome.

Overdose

Topical overdose is unlikely due to minimal systemic absorption. However, excessive application may lead to increased local irritation, redness, or discomfort. If accidentally ingested, seek medical attention immediately; symptoms may include nausea, vomiting, diarrhea, or dizziness. In case of ocular overdose, rinse the eye gently with warm water to remove excess ointment.

Storage

Store at controlled room temperature (15–30°C or 59–86°F). Do not freeze. Keep the tube tightly closed and away from direct light and moisture. Keep out of reach of children and pets. Do not use after the expiration date printed on the packaging. Discard any medication that appears discolored or shows signs of contamination.
