Proscar (finasteride) is a prescription medication specifically formulated for the management of symptomatic benign prostatic hyperplasia (BPH) in men. As a 5-alpha-reductase inhibitor, it works at a hormonal level to target the underlying cause of prostate enlargement, offering a non-surgical approach to improving urinary function and quality of life. This long-term treatment option is supported by extensive clinical research and is a cornerstone in the urological management of BPH.
Features
- Active Ingredient: Finasteride 5 mg
- Drug Class: 5-alpha-reductase inhibitor (Type II)
- Presentation: Film-coated, octagonal-shaped tablets
- Mechanism of Action: Inhibits the conversion of testosterone to dihydrotestosterone (DHT)
- Prescription Status: Rx-only medication
Benefits
- Reduces Prostate Volume: Significantly decreases the size of the enlarged prostate gland by lowering intraprostatic DHT levels, the primary driver of hyperplasia.
- Improves Urinary Flow: Leads to a measurable increase in urinary flow rate (Qmax), reducing the obstructive symptoms caused by the prostate pressing on the urethra.
- Alleviates Lower Urinary Tract Symptoms (LUTS): Provides sustained relief from bothersome symptoms such as hesitancy, weak stream, straining, nocturia, urgency, and incomplete bladder emptying.
- Lowers Risk of Acute Urinary Retention (AUR): Reduces the long-term probability of developing AUR, a painful condition requiring catheterization.
- Decreases the Need for Surgery: Effective management with Proscar can significantly reduce the likelihood of requiring invasive surgical procedures like transurethral resection of the prostate (TURP).
- Long-Term Management: Offers a well-tolerated, oral solution for the chronic management of BPH.
Common use
Proscar is indicated for the treatment of symptomatic benign prostatic hyperplasia (BPH) in men with an enlarged prostate. It is used to improve urinary flow and to reduce the risk of acute urinary retention and the need for prostate-related surgery. It is important to note that while Proscar is also the active ingredient in Propecia® (finasteride 1 mg), which is used for male pattern hair loss, Proscar (5 mg) is not indicated or approved for the treatment of hair loss.
Dosage and direction
The recommended dosage of Proscar is one 5 mg tablet taken orally once a day, with or without food. The tablet should be swallowed whole and not crushed or broken due to the potential for absorption of the active ingredient through the skin, which is a particular concern for pregnant women. A period of at least six months of treatment is usually necessary to assess whether an individual will respond to therapy. Treatment is typically long-term, and continued use is necessary to sustain therapeutic benefits. Discontinuation of treatment leads to a gradual return to pre-treatment prostate size and symptoms.
Precautions
- Pregnancy Warning: Finasteride is contraindicated in women who are or may become pregnant. The crushed or broken tablet must not be handled by women who are pregnant or who may become pregnant due to the risk of absorption through the skin and the potential for causing abnormalities of the external genitalia in a male fetus.
- Prostate Cancer Evaluation: Prior to initiating therapy with Proscar, patients should be evaluated to rule out other urological diseases, including carcinoma of the prostate. Proscar causes a decrease in serum prostate-specific antigen (PSA) levels; therefore, a new baseline PSA should be established after six months of treatment. Any sustained increase in PSA while on Proscar should be carefully evaluated, as it may signal the presence of prostate cancer.
- Blood Donation: Men being treated with Proscar should not donate blood until at least one month has passed after their final dose to prevent administration of finasteride to a pregnant female transfusion recipient.
- Hepatic Impairment: Caution should be exercised in the administration of Proscar in those patients with liver function abnormalities, as finasteride is metabolized extensively in the liver.
Contraindications
Proscar is contraindicated in the following scenarios:
- Hypersensitivity to finasteride or any other component of the formulation.
- Use in women, particularly those who are pregnant or of childbearing potential.
- Use in pediatric patients.
Possible side effect
As with all medications, Proscar can cause side effects, although not everybody gets them. The majority of side effects are related to the drug’s endocrine effects and are generally mild and transient.
Common (≥1/100):
- Decreased libido (sexual desire)
- Erectile dysfunction
- Ejaculation disorder (e.g., decreased ejaculate volume)
Uncommon (≥1/1,000 to <1/100):
- Breast enlargement (gynecomastia) and/or tenderness
- Skin rash
Rare:
- Testicular pain
- Depression
- Allergic reactions, including lip swelling and skin rash
A small number of men may experience persistent sexual dysfunction (e.g., erectile dysfunction, diminished libido, ejaculatory disorders) that continues after discontinuation of treatment. Patients should discuss any persistent side effects with their physician.
Drug interaction
Formal drug interaction studies have not revealed any clinically significant interactions. However, caution is advised with the following:
- Therapeutic Interactions: Although not observed in studies, potential interactions are theoretically possible with other drugs that are extensively metabolized by the CYP3A4 enzyme system.
- Laboratory Tests: As noted, Proscar reduces serum PSA levels by approximately 50% in patients with BPH. This reduction should be considered when evaluating PSA laboratory values for prostate cancer screening. For more accurate monitoring, PSA values should be doubled for comparison to normal ranges in untreated men.
Missed dose
If a dose is missed, it should be taken as soon as remembered. However, if it is almost time for the next dose, the missed dose should be skipped, and the regular dosing schedule resumed. Do not take a double dose to make up for a forgotten tablet.
Overdose
In clinical studies, doses of finasteride up to 400 mg per day have been administered without significant adverse effects. There is no specific antidote for finasteride overdose. In the event of a suspected overdose, symptomatic and supportive treatment should be instituted as appropriate.
Storage
- Store Proscar tablets at room temperature (15°C to 30°C or 59°F to 86°F).
- Keep the medication in its original container, tightly closed, and out of the reach of children and pets.
- Protect from light and moisture.

Reviews
“After six months on Proscar, my urinary symptoms have improved dramatically. My flow is stronger, and I’m no longer getting up multiple times at night. It has given me a significant quality-of-life improvement.” – John D., 68
“As a urologist, I have prescribed Proscar for over two decades. It remains a first-line, evidence-based option for appropriate patients with BPH. The key is patient education on the timeline for effect and potential side effects.” – Dr. Evan R., Urologist
“I experienced some initial side effects related to sexual function, but they subsided after the first few months. The trade-off for being able to sleep through the night has been worth it for me.” – Robert T., 61
“The reduction in PSA was initially concerning until my doctor explained it was an expected effect of the medication. We established a new baseline, and I continue to be monitored annually.” – Michael S., 72
