Common use
Prevacid is indicated for the short-term treatment (4–8 weeks) of active duodenal ulcers, active benign gastric ulcers, and erosive esophagitis. It is also used for maintaining healing of erosive esophagitis and for the long-term treatment of pathological hypersecretory conditions, including Zollinger-Ellison syndrome. Additionally, it is prescribed as part of a combination regimen with antibiotics such as amoxicillin and clarithromycin for the eradication of Helicobacter pylori to reduce the risk of ulcer recurrence. Many patients use it for symptomatic relief of frequent heartburn and GERD.
Dosage and direction
The recommended adult dosage for healing erosive esophagitis is 30 mg once daily for up to 8 weeks. For maintenance therapy, 15 mg once daily is typical. For active duodenal ulcers, the dose is 15 mg once daily for 4 weeks. For H. pylori eradication, a triple therapy regimen of Prevacid 30 mg twice daily plus amoxicillin 1 g twice daily and clarithromycin 500 mg twice daily for 10–14 days is common. Prevacid should be taken before a meal, preferably in the morning. Capsules must be swallowed whole; do not crush or chew. For patients unable to swallow capsules, the contents may be sprinkled on a tablespoon of applesauce and swallowed immediately without chewing. Orally disintegrating tablets should be placed on the tongue and allowed to disintegrate, then swallowed with or without water.
Precautions
Patients should inform their healthcare provider of any liver disease, as dosage adjustment may be necessary. Long-term use (more than 3 years) may lead to vitamin B12 deficiency; monitoring is advised. There is an increased risk of bone fractures with prolonged, high-dose PPI therapy. Hypomagnesemia may occur with long-term treatment; monitor magnesium levels before and periodically during therapy. Prevacid may mask symptoms of gastric malignancy; appropriate diagnostic evaluation should be conducted if alarm symptoms (e.g., unintentional weight loss, recurrent vomiting, dysphagia) occur. Use with caution in patients with osteoporosis or those receiving concomitant digoxin or diuretics.
Contraindications
Prevacid is contraindicated in patients with known hypersensitivity to lansoprazole or any component of the formulation. Concomitant use with rilpivirine-containing products is contraindicated due to the potential for decreased rilpivirine plasma concentrations and loss of virologic response. It should not be used in patients taking drugs that are highly dependent on CYP2C19 for clearance (e.g., clopidogrel) due to potential reduced efficacy.
Possible side effect
Common side effects include headache, diarrhea, abdominal pain, and nausea. Less frequently, patients may experience constipation, dizziness, or rash. Serious side effects, though rare, may include severe diarrhea due to Clostridium difficile, acute interstitial nephritis, cutaneous lupus erythematosus, or systemic lupus erythematosus. Long-term use has been associated with hypomagnesemia, vitamin B12 deficiency, and increased risk of fractures. Patients should seek medical attention if they experience signs of an allergic reaction, such as rash, itching, swelling, severe dizziness, or trouble breathing.
Drug interaction
Prevacid may interact with drugs whose absorption is pH-dependent, such as ketoconazole, iron salts, and digoxin, potentially reducing their efficacy. It can inhibit CYP2C19 and may increase concentrations of drugs metabolized by this enzyme, such as phenytoin or warfarin (monitor INR). Concomitant use with methotrexate may increase methotrexate levels. PPIs may reduce the antiplatelet effect of clopidogrel. Avoid use with atazanavir and nelfinavir due to reduced antiretroviral concentrations.
Missed dose
If a dose is missed, take it as soon as remembered unless it is almost time for the next dose. Do not double the dose to catch up. Resume the regular dosing schedule. Consistency in timing is important for optimal acid suppression.
Overdose
There is limited experience with lansoprazole overdose. Expected manifestations would be consistent with the drug’s pharmacologic effects, such as hypersecretion of gastric acid. Supportive care and symptomatic treatment are recommended. Dialysis is not likely to be effective due to high protein binding. In case of suspected overdose, contact a poison control center or seek emergency medical attention.
