Synonyms

Nizoral (ketoconazole) is a potent, broad-spectrum antifungal medication available in both shampoo and cream formulations. It is a first-line therapeutic choice for dermatologists worldwide, targeting the underlying fungal causes of dandruff, seborrheic dermatitis, and various cutaneous mycoses. Its primary active ingredient works by inhibiting the synthesis of ergosterol, a critical component of fungal cell membranes, leading to fungal cell death. This product card provides a comprehensive, expert-level overview of its appropriate use, mechanisms, and safety profile for healthcare professionals and informed patients.

Features

  • Active Ingredient: Ketoconazole 20 mg/g (2%) in shampoo and cream formulations.
  • Mechanism of Action: Belongs to the imidazole class of antifungals; acts by inhibiting the cytochrome P-450-dependent synthesis of ergosterol, a vital component of the fungal cell membrane.
  • Formulations: Available as a topical cream for skin application and a medicated shampoo for scalp conditions.
  • Spectrum of Activity: Demonstrates broad-spectrum fungicidal and fungistatic activity against Malassezia species (e.g., M. globosa, M. restricta), dermatophytes (Trichophyton, Microsporum, Epidermophyton), and yeasts (Candida species).
  • Additional Action: The shampoo formulation exhibits anti-inflammatory and anti-seborrheic properties, helping to reduce scaling, itching, and redness.

Benefits

  • Targets the Root Cause: Effectively eliminates the fungal overgrowth responsible for symptoms, rather than merely suppressing flaking and itching.
  • Rapid Symptom Relief: Clinically proven to significantly reduce scaling, itching, and redness associated with dandruff and seborrheic dermatitis often after just a few uses.
  • Sustained Efficacy: Provides long-lasting control of fungal conditions with proper maintenance use, preventing frequent recurrence.
  • Dual-Action Shampoo: Offers both potent antifungal and anti-inflammatory benefits, making it highly effective for inflammatory scalp conditions.
  • Well-Tolerated Profile: When used as directed, topical ketoconazole is generally well-tolerated with a low incidence of systemic side effects.

Common use

Nizoral is indicated for the treatment and management of fungal infections caused by susceptible organisms.

  • Shampoo: Primarily used for the treatment of dandruff (pityriasis capitis) and seborrheic dermatitis of the scalp. It is also used off-label as an adjunctive treatment for tinea versicolor (pityriasis versicolor) on the body and for scalp psoriasis to reduce scaling.
  • Cream: Used for the topical treatment of cutaneous candidiasis, tinea corporis (ringworm), tinea cruris (jock itch), tinea pedis (athlete’s foot), and tinea versicolor.

Dosage and direction

For Nizoral Shampoo (2%):

  • Dandruff and Seborrheic Dermatitis: Wet hair thoroughly. Apply sufficient shampoo to produce a rich lather and massage over the entire scalp. Leave on for 3–5 minutes. Rinse thoroughly. Repeat application twice weekly for 2–4 weeks, spacing uses by at least 3 days. For maintenance, use once every 1–2 weeks or as directed by a physician.
  • Tinea Versicolor (off-label body wash): Apply to affected areas on the body, lather, and leave on for 5 minutes. Rinse thoroughly. Use daily for up to 2 weeks.

For Nizoral Cream (2%):

  • Apply a sufficient amount to cover the affected area and surrounding skin once or twice daily, as directed. For tinea pedis, treatment typically continues for 4 weeks; for other infections, 2 weeks is often sufficient. Clinical improvement usually occurs within the first week. Continue application for the full prescribed duration to prevent relapse.

Precautions

  • For external use only. Avoid contact with eyes, nose, mouth, and other mucous membranes. If contact occurs, rinse thoroughly with water.
  • Do not use on broken or severely inflamed skin unless directed by a healthcare provider.
  • Discontinue use and consult a doctor if condition worsens or does not improve after 4 weeks of regular use.
  • Inform your doctor if you are pregnant, planning to become pregnant, or are breastfeeding before using this medication.
  • Although systemic absorption is low, caution is advised in patients with known liver dysfunction.

Contraindications

Nizoral is contraindicated in patients with a known hypersensitivity to ketoconazole, any other imidazole antifungals, or any of the inactive ingredients in the formulation.

Possible side effect

Topical application is associated with a low risk of side effects, which are typically localized and mild.

  • Common (affecting >1% of users): Local skin irritation, burning sensation, stinging, itching, or dryness at the application site.
  • Uncommon: Allergic contact dermatitis, skin rash, pruritus, oiliness, or dryness of the hair and scalp (with shampoo).
  • Rare: Severe irritation, blistering, or peeling. Systemic effects are extremely rare due to minimal absorption but could include headache or dizziness.

Drug interaction

The risk of systemic drug interactions with topical Nizoral is very low due to minimal absorption into the bloodstream. However, caution is theoretically advised when using with drugs that are extensively metabolized by the CYP3A4 enzyme system, though this is a greater concern with oral ketoconazole.

Missed dose

Apply the missed dose as soon as you remember. If it is almost time for the next application, skip the missed dose and resume the usual dosing schedule. Do not apply a double dose to make up for a missed one.

Overdose

Topical overdose is unlikely. Excessive application may lead to increased incidence of local adverse effects such as severe skin irritation. If the product is ingested orally, seek immediate medical attention or contact a poison control center. Symptoms of ingestion may include nausea, vomiting, and abdominal pain.

Storage

  • Store at room temperature between 20°C to 25°C (68°F to 77°F).
  • Keep the container tightly closed and away from direct light and moisture.
  • Do not freeze.
  • Keep out of reach of children and pets.
