Common use

Dosage and direction
Dosing is typically based on ideal body weight, generally not exceeding 5.0 mg/kg actual body weight daily. For rheumatoid arthritis and lupus, the usual adult dose is 400-600 mg daily initially, often reduced to a maintenance dose of 200-400 mg daily. Tablets should be taken with food or milk to minimize gastrointestinal upset. Regular ophthalmologic monitoring is required due to potential retinal toxicity.
Precautions
Patients should undergo baseline and annual ophthalmologic examinations including visual fields and SD-OCT. Renal and hepatic function should be monitored periodically. Blood glucose levels should be observed in diabetic patients as hydroxychloroquine may enhance the effects of insulin and oral hypoglycemics. Use with caution in patients with pre-existing retinal disease, hepatic impairment, or glucose-6-phosphate dehydrogenase deficiency.
Contraindications
Hydroxychloroquine is contraindicated in patients with known hypersensitivity to 4-aminoquinoline compounds. It should not be used in patients with pre-existing macular changes or visual field defects attributable to 4-aminoquinolines. Contraindicated in combination with other drugs known to cause retinal toxicity.
Possible side effects
Common side effects include gastrointestinal symptoms (nausea, diarrhea, abdominal cramps), headache, and dizziness. More serious but rare adverse effects include irreversible retinal toxicity, cardiomyopathy, and neuromyotoxicity. Cutaneous reactions ranging from rash to severe reactions such as Stevens-Johnson syndrome may occur. Pigmentary changes in skin and mucous membranes have been reported with long-term use.
Drug interactions
Hydroxychloroquine may potentiate the effects of digoxin and insulin. Concomitant use with hepatotoxic drugs may increase risk of liver damage. Antacids and kaolin may reduce absorption. May enhance the effects of QT-prolonging agents. Caution advised with medications that affect cytochrome P450 enzymes.
Missed dose
If a dose is missed, it should be taken as soon as remembered unless it is nearly time for the next dose. Do not double the dose to make up for a missed one. Maintain regular dosing schedule to ensure consistent therapeutic levels.
Overdose
Overdose can be fatal, with symptoms including headache, visual disturbances, cardiovascular collapse, convulsions, and hypokalemia. Immediate medical attention is required. Treatment is supportive and symptomatic, with particular attention to cardiac and respiratory status. Gastric lavage may be beneficial if performed soon after ingestion.
Storage
Store at room temperature (15-30°C/59-86°F) in a tight, light-resistant container. Keep out of reach of children and pets. Do not use after expiration date printed on packaging.
