Foracort Inhaler is a prescription-based combination inhaler designed for the maintenance treatment of asthma and chronic obstructive pulmonary disease (COPD). It contains two active ingredients: Budesonide, a corticosteroid that reduces inflammation in the airways, and Formoterol, a long-acting bronchodilator that relaxes the muscles around the airways. This synergistic formulation provides both anti-inflammatory and bronchodilatory effects, offering comprehensive management of respiratory symptoms, improving lung function, and reducing the frequency of exacerbations. It is intended for regular use and should be administered under medical supervision to ensure optimal therapeutic outcomes.
Features
- Contains Budesonide (corticosteroid) and Formoterol (long-acting beta2-agonist)
- Available in multiple strength variants (e.g., 100/6 mcg, 200/6 mcg, 400/6 mcg per puff)
- Delivered via a pressurized metered-dose inhaler (pMDI)
- Designed for twice-daily maintenance therapy
- Built-in dose counter to track remaining medication
- Requires prescription; not for immediate relief of acute bronchospasm
Benefits
- Provides dual-action therapy targeting both inflammation and bronchoconstriction
- Reduces the frequency and severity of asthma and COPD exacerbations
- Improves overall lung function and exercise tolerance
- Enhances quality of life by enabling better day-to-day symptom control
- May reduce the need for rescue inhalers when used consistently as prescribed
- Helps prevent long-term airway remodeling and disease progression
Common use
Foracort Inhaler is commonly prescribed for the maintenance treatment of asthma in patients who require both inhaled corticosteroid and long-acting bronchodilator therapy. It is also indicated for symptomatic management of COPD, including chronic bronchitis and emphysema, in patients with a history of exacerbations. It is not intended for the relief of acute bronchospasm or as a rescue medication during asthma attacks.
Dosage and direction
The dosage of Foracort Inhaler must be individualized based on disease severity and patient response. The usual recommended dose for asthma and COPD is 1 or 2 inhalations twice daily (morning and evening). Patients should be instructed on proper inhaler technique to ensure effective drug delivery: shake the inhaler well before each use, exhale fully, place the mouthpiece between the lips, inhale deeply and slowly while pressing the canister, hold breath for 5–10 seconds, and exhale slowly. Rinsing the mouth with water after each use is advised to reduce the risk of oral candidiasis.
Precautions
- Not for use in acute asthma attacks or status asthmaticus.
- Caution advised in patients with cardiovascular disorders, diabetes, osteoporosis, or glaucoma.
- Systemic effects of corticosteroids may occur, especially at high doses or with prolonged use.
- Monitor for signs of adrenal insufficiency during stress or after switching from systemic steroids.
- Avoid sudden discontinuation; taper under medical guidance if switching therapy.
- Use with caution in patients with thyrotoxicosis or hypokalemia.
- Pregnancy and lactation: use only if clearly needed and under medical supervision.
Contraindications
- Hypersensitivity to Budesonide, Formoterol, or any component of the formulation.
- Primary treatment of status asthmaticus or other acute episodes of asthma or COPD where intensive measures are required.
- Not recommended for children under 6 years of age.
Possible side effects
Common side effects may include:
- Headache
- Tremor
- Palpitations
- Hoarseness or dysphonia
- Oral candidiasis (thrush)
- Throat irritation or coughing
Less common but serious side effects may include:
- Cardiovascular effects (tachycardia, arrhythmias)
- Paradoxical bronchospasm
- Signs of systemic corticosteroid effects (e.g., adrenal suppression, hypercorticism)
- Hypokalemia
- Allergic reactions (rash, angioedema)
Drug interactions
- Concomitant use with other beta-adrenergic agents may potentiate cardiovascular effects.
- Ketoconazole, itraconazole, or other CYP3A4 inhibitors may increase Budesonide exposure.
- Diuretics or corticosteroids may enhance the risk of hypokalemia.
- MAO inhibitors and tricyclic antidepressants may potentiate the effects of Formoterol.
- Beta-blockers may antagonize the effects of Formoterol and produce severe bronchospasm.
Missed dose
If a dose is missed, it should be taken as soon as remembered unless it is almost time for the next dose. Do not double the dose to make up for a missed one. Resume the regular dosing schedule.
Overdose
Overdose may lead to excessive beta-adrenergic stimulation, including tachycardia, arrhythmias, tremor, headache, and hypokalemia. Corticosteroid overdose may result in hypercorticism. Symptomatic and supportive treatment is recommended. Cardiac monitoring and serum potassium level assessment may be necessary. There is no specific antidote.
Storage
Store at room temperature (15–30°C). Keep the canister away from direct heat or sunlight. Do not puncture, break, or incinerate the inhaler, even when empty. Keep out of reach of children.
