Common use

Cozaar is primarily indicated for the treatment of hypertension in adults and pediatric patients 6 years and older. It is also used to reduce the risk of stroke in patients with hypertension and left ventricular hypertrophy. Additionally, it is prescribed for the treatment of diabetic nephropathy with an elevated serum creatinine and proteinuria in patients with type 2 diabetes and a history of hypertension.

Dosage and direction

The recommended starting dose of Cozaar for most adult patients with hypertension is 50 mg once daily. The dosage can be increased to 100 mg once daily based on blood pressure response. For hypertensive patients with intravascular volume depletion, a starting dose of 25 mg is recommended. For the treatment of diabetic nephropathy, the usual starting dose is 50 mg once daily, which can be increased to 100 mg once daily based on tolerability and blood pressure response. Tablets may be taken with or without food. Dosage in pediatric hypertensive patients (6 years and older) is based on weight.

Precautions

Patients should be monitored for hypotension, especially after the initial dose or during dose titration. Renal function should be assessed prior to initiation and periodically thereafter, particularly in patients with renal impairment. Serum potassium levels should be monitored, especially in patients with renal impairment, diabetes, or those taking potassium supplements or potassium-sparing diuretics. Use with caution in patients with hepatic impairment, as losartan exposure is increased. Cozaar should not be used during pregnancy due to the risk of fetal harm.

Contraindications

Cozaar is contraindicated in patients who are hypersensitive to any component of this product. It is also contraindicated in patients with a history of angioedema related to previous treatment with an ARB. Do not co-administer with aliskiren in patients with diabetes.

Possible side effect

Common side effects may include dizziness, upper respiratory infection, stuffy nose, back pain, and diarrhea. Less common but more serious side effects can include symptoms of high potassium levels (such as muscle weakness, slow or irregular heartbeat), kidney problems (change in the amount of urine), and severe allergic reactions (rash, itching/swelling, severe dizziness, trouble breathing).

Drug interaction

Cozaar may interact with other medications that increase potassium levels, such as potassium supplements, salt substitutes containing potassium, or potassium-sparing diuretics. Nonsteroidal anti-inflammatory drugs (NSAIDs) may reduce the antihypertensive effect and increase the risk of renal impairment. Concurrent use with lithium may increase lithium levels and toxicity. Use with aliskiren is contraindicated in patients with diabetes.

Missed dose

If a dose is missed, it should be taken as soon as remembered. However, if it is almost time for the next dose, the missed dose should be skipped and the regular dosing schedule resumed. Do not double the dose to make up for a missed one.

Overdose

Symptoms of overdose may include hypotension and tachycardia; bradycardia could occur from parasympathetic (vagal) stimulation. If overdose occurs, supportive treatment should be instituted. The patient should be placed in supine position and salt and volume depletion should be corrected. Losartan and its active metabolite cannot be removed by hemodialysis.

Storage

Store at room temperature (20-25°C or 68-77°F) in a dry place, protected from light and moisture. Keep out of reach of children and pets. Do not use after the expiration date printed on the packaging.
