Common use
Cepmox is indicated for the treatment of bacterial infections including community-acquired pneumonia, acute bacterial sinusitis, otitis media, skin and soft tissue infections, urinary tract infections, and intra-abdominal infections. It is also used for prophylactic coverage in surgical procedures involving the gastrointestinal tract. Off-label use may include dental infections and bite wounds, following susceptibility testing.
Dosage and direction
Adults and children ≥40 kg:
One tablet (500mg/125mg) every 12 hours for 7–14 days, depending on infection severity.
Children <40 kg:
20–40 mg/kg/day (based on amoxicillin component) divided every 12 hours.
Take with food to enhance absorption and reduce gastrointestinal upset. Complete the full course even if symptoms improve earlier. For severe infections or those caused by less susceptible organisms, dosage may be increased to one tablet every 8 hours.
Precautions
- Use with caution in patients with hepatic impairment; monitor liver enzymes periodically
- Assess renal function before initiation and adjust dose if creatinine clearance is <30 mL/min
- May cause antibiotic-associated colitis; discontinue if diarrhea develops
- Not recommended during pregnancy unless potential benefit justifies potential risk
- Use in lactating women only if clearly needed; amoxicillin excretes in breast milk

Contraindications

Rare: pseudomembranous colitis, hepatitis, eosinophilia, Stevens-Johnson syndrome.
Discontinue immediately if signs of hypersensitivity reaction occur.
Drug interaction
- Probenecid decreases renal tubular secretion of amoxicillin, increasing serum concentration
- Concurrent use with oral anticoagulants may enhance anticoagulant effect
- Reduces efficacy of oral contraceptives; advise alternative contraception during therapy
- Allopurinol increases incidence of skin rash
- May alter intestinal flora, reducing efficacy of estrogen-containing therapies
Missed dose
Take the missed dose as soon as remembered unless it is almost time for the next dose. Do not double the dose to catch up. Resume the regular dosing schedule. Consistent dosing is critical to maintain therapeutic blood levels.
Overdose
Symptoms may include nausea, vomiting, diarrhea, electrolyte imbalance, or crystalline. Gastrointestinal symptoms are managed supportively. Hemodialysis can remove both amoxicillin and clavulanic acid. Maintain hydration and electrolyte balance. Contact poison control or seek emergency medical attention.
Storage
Store at room temperature (15–30°C) in the original container. Protect from light and moisture. Keep out of reach of children. Do not use after the expiration date printed on the packaging. Do not transfer to alternative containers.
